What To Bring When you schedule your appointment, you will receive information about what to bring and how to prepare for your visit, if necessary. Although a member of our staff will tell you specifically what you will need to bring, information we typically require includes:
· A list of your current medications (prescribed and over-the-counter), vitamins, and any herbal supplements that you might be taking
· Your insurance information
· A referral, if your health insurance provider requires it
· Completed new patient paperwork, if necessary (Click here to download new patient paperwork. Note: You must have access to a printer.)
· Your co-payment or deductible: You may be asked to pay for services not covered by insurance. Please bring cash, check, or major credit card (Visa, MasterCard, or American Express).
Be prepared to provide a urine sample during your visit. Billing We have professional billing staff in each of our medical offices to answer any questions you might have. For your convenience, we accept cash, check, Visa, MasterCard, and American Express. If you think your medical problem is life threatening, call 911 immediately.
