Referrals, Billing and Insurance

If your insurance requires referrals, you must obtain the initial referral before your first visit. You should also know how many follow-up visits are allowed as well as the referral expiration date.


We have professional billing staff in each of our medical offices to answer any questions you have. For your convenience, we accept cash, check, MasterCard, Visa and American Express. We require payment on any outstanding balances due at the time of service.

Because insurance coverage varies, it is important that you understand your individual health plan and what it covers, including deductibles, coinsurance and copays. Call the customer service number on the back of your insurance card or contact your employer's benefits department to verify your coverage and for any questions regarding your health insurance plan. Our staff will work with your insurance company to confirm eligibility and coverage for procedures and/or services provided. 


Please bring your insurance card(s) to each visit. We accept all public and most private insurances. We will confirm that we accept your insurance when you make an appointment.

No Insurance

If you have an urgent need to be treated for a urologic condition our staff will help you understand your options and the process for obtaining coverage. Payment plans and/or services from non-profit organizations may also help to lessen your financial concerns. Talk to our staff in the office location convenient to you. We try to help so that you can focus on healing.
